Pengertian Fobia

Fobia adalah rasa takut berlebihan terhadap sesuatu. Ketakutan tersebut dapat timbul saat menghadapi situasi, berada di suatu tempat, atau ketika melihat hewan tertentu. Dalam kondisi fobia yang parah, penderitanya akan berusaha menghindar dari objek yang dapat memicu ketakutan.

Fobia sebenarnya termasuk ke dalam penyakit gangguan kecemasan. Kondisi ini dapat membuat penderitanya depresi, panik, serta membatasi kegiatan.

Fobia bisa bersifat spesifik atau kompleks. Contoh-contoh fobia spesifik, di antaranya adalah takut terhadap kedalaman air, ketinggian, hewan, dokter, jarum suntik, darah, atau takut tertular penyakit seksual. Sedangkan contoh fobia kompleks, di antaranya adalah takut terhadap situasi sosial, takut berbicara di depan umum, atau takut berada di ruang terbuka.

Kebanyakan kasus fobia spesifik dialami oleh penderitanya sejak masa kanak-kanak atau remaja. Sedangkan fobia kompleks umumnya mulai berkembang ketika penderitanya memasuki kehidupan dewasa.

Gejala Fobia

Tanda fobia pada diri seseorang dapat mudah dikenali dari reaksi takut berlebihan yang diperlihatkannya ketika melihat objek atau menghadapi situasi tertentu. Selain rasa takut yang berlebihan, fobia juga bisa disertai dengan serangan panik yang ditandai dengan:

  • Disorientasi atau bingung.
  • Pusing dan sakit kepala.
  • Mual.
  • Dada terasa sesak dan nyeri.
  • Sesak napas.
  • Detak jantung meningkat.
  • Tubuh gemetar dan berkeringat.
  • Telinga berdenging.
  • Sensasi ingin selalu buang air kecil.
  • Mulut terasa kering.
  • Menangis terus-menerus dan takut ditinggal sendirian (terutama pada anak-anak).

Penyebab Fobia

Hingga kini penyebab fobia belum diketahui secara jelas. Meski begitu, ada beberapa faktor yang diduga kuat dapat memicu  kondisi ini, di antaranya:

  • Peristiwa traumatis atau pengalaman buruk.Fobia sering dikaitkan dengan  peristiwa traumatis yang dialami sebelumnya atau pengalaman buruk pada masa kecil. Misalnya, seseorang yang pernah terkurung saat masih kecil cenderung takut terhadap ruang tertutup ketika beranjak dewasa.
  • Perubahan fungsi otak. Beberapa fobia spesifik dapat disebabkan oleh perubahan yang terjadi pada fungsi otak.
  • Genetik dan lingkungan. Fobia dapat terjadi karena pengaruh dari lingkungan atau keluarga. Contohnya, seseorang cenderung akan mengalami fobia jika dibesarkan oleh orang tua yang sering mengalami kecemasan.

Diagnosis dan Pengobatan Fobia

Fobia biasanya dapat mudah terdiagnosis oleh dokter dari gejala-gejala yang mengarah pada kondisi tersebut, dengan diperkuat oleh riwayat penyakit (termasuk kejiwaan), riwayat penggunaan obat, dan riwayat kehidupan sosial pasien.

Penanganan terhadap fobia dapat dilakukan melalui terapi psikologi, salah satunya yang efektif adalah terapi perilaku kognitif. Terapi ini membantu pasien mengubah cara pandang dan cara bersikap terhadap suatu masalah. Dalam kasus fobia, ahli terapi akan membantu pasien mengatasi rasa takut melalui teknik pemaparan atau desentisasi. Dengan teknik pemaparan terhadap benda atau suasana yang ditakuti, rasa takut diharapkan dapat berkurang secara bertahap sehingga pada akhirnya pasien dapat mengendalikan fobia yang dialami. Contohnya, adalah pada pasien yang mengalami fobia terhadap ular. Awalnya, pasien akan diminta untuk membaca tulisan tentang ular, lalu diperlihatkan gambar hewan  tersebut. Tahapan berikutnya adalah dengan mengunjungi kandang ular, yang dilanjutkan dengan memegang reptil tersebut secara langsung.

Di samping teknik tersebut, ahli terapi juga akan mengajarkan pasien teknik untuk mengendalikan diri. Misalnya, melalui teknik relaksasi untuk membantu mengatur ketenangan dan pernapasan, atau teknik visualisasi untuk membayangkan keberhasilan mengatasi situasi.

Hasil yang lebih efektif akan terlihat saat beberapa teknik terapi dipadukan dengan ditunjang oleh penerapan gaya hidup sehat. Misalnya beristirahat secara cukup, mengonsumsi makanan sehat secara teratur, dan rajin berolah raga.

Selain melalui terapi, gejala fobia juga dapat diredakan dengan obat-obatan. Kendati demikian, obat biasanya hanya diberikan untuk jangka waktu pendek. Contoh obat yang kemungkinan  diresepkan dokter dalam kasus fobia adalah:

  • Penghambat pelepasan serotonin (SSRIs). Obat ini bekerja dengan cara memengaruhi salah satu hormon transmiter di dalam otak, yaitu hormon serotonin, berperan dalam menciptakan dan mengatur suasana hati.
  • Penghambat beta (beta blockers). Obat yang biasanya digunakan untuk mengatasi hipertensi dan gangguan jantung  ini  diberikan untuk menghambat reaksi-reaksi yang muncul dari stimulasi adrenalin akibat  rasa cemas, seperti suara dan tubuh gemetar, jantung berdebar, atau tekanan darah meningkat.
  • Benzodiazepine. Obat ini diberikan untuk mengatasi kecemasan dalam tingkat yang parah. Biasanya pemberian benzodiazepine akan dikurangi secara bertahap seiring membaiknya kondisi guna menghindari ketergantungan.

Hipnoterapi untuk Fobia

Teknik relaksasi dengan melakukan perubahan terhadap mindset pasien/ klien sangatlah efektif dalam menangani kasus fobia. Teknik ini dikenal dengan nama Hipnoterapi.

Dalam kondisi kedalaman tertentu yang memungkinkan terapis dapat bekerja dengan baik, yang dinamakan profound somnambulism pasien/ klien akan dibimbing untuk menyusuri kembali waktu di mana fobia tersebut muncul untuk pertama kalinya dalam kehidupan pasien/ klien dan terapis membimbing klien untuk menghilangkan fobia yang dialaminya. Beberapa teknik hipnoterapi bahkan bisa menyelesaikan permasalahan fobia tertentu dalam hitungan 3-5 menit. Hasilnya sangat memuaskan dan permanen.
